How to complete a Regional Traveler affidavit form? 

If you plan to travel to Guatemala by air and arrival is coming up soon (no earlier than 15 days) make sure you fill out the updated Electronic Traveler Affidavit. 

Be sure to complete this form, for all passengers flying by air to prevent any complications during your journey.

1. Complete the Form Online

Fill out the questionnaire with your names, email, age, and passport number.

2. Pay through a Secure page

Have your credit card ready to pay the Guatemala Travel Affidavit fee.

3. Receive Your QR & Travel

The Guatemala Travel Affidavit along with its QR code will be sent shortly to your email address.
